Can a coach make two trips to the mound during the same at bat? What happens to who and why? What constitutes a trip?

Yes. Pro rules. Under usual circumstances, the second trip to the mound requires that the pitcher be removed from the game. The reason for the rule would be the need to put a limit on conferences that delay the game. A "trip" therefore would be any communication between a coach and the pitcher that would delay the game. That would include a coach talking to the catcher, and then sending the catcher to the pitching mound to relay some information. It would exclude the coach going to the pitcher during an offensive conference.
Under unusual circumstances, a second trip to the mound during the same at bat might not result in the pitcher leaving the game, i.e. he may be visiting a relief pitcher who is still pitching to the original batter.

Er....uh....you mean according to the RULES! and not my faulty memory? Allow me to re-state: Two trips to mound, same pitcher, same inning, remove pitcher. Two trips to mound, same pitcher, same inning, same batter, remove coach and then remove pitcher after completion of at bat (after having warned the coach he can't do it). Exception: two trips to mound, same pitcher, same inning, same at bat but different batter, remove the pitcher after completion of that at bat.
